On Tuesday, June 9th, doctors from The Franciscan Physician Network of Orthopedics & Sports Medicine will be in attendance at the MCHS Gym from 5:30pm-8pm providing the opportunity to get a student-athlete's 2026-2027 IHSAA Sports Physical completed. This opportunity is open to all rising 7th-12th grade students. The cost is only $25 per physical. Please park in the rear of the building and enter through Door J under the Welcome to the Wolves Den sign. Michigan City Athletic Trainers Sarah Brinckman (sbrinckman@mcas.k12.in.us) and Molly Perales (mperales@mcas.k12.in.us) will oversee operations and the physicals will be conducted by staff members of The Franciscan Physician Network of Orthopedics & Sports Medicine who provide services for Michigan City Student-Athletes. PLEASE NOTE: A fully completed Health History Form must be presented to the physician BEFORE the physician will sign off on the competed physical form. If this has already been completed in FinalForms please bring a printed copy with you (or they can be printed on site). Please make sure this information is entered in FinalForms BEFORE your appointment window. In order to help cut wait times, student-athletes should sign up for the designated time slots. In order to be valid to participate in Michigan City Athletics, the physical exam information must be completed on the IHSAA Pre-Participation Physical Evaluation form which is available below.
